The Life is Good Kids Design-a-Thon

PARENTAL CONSENT, RELEASE & LICENSE

 

This Parental Consent, Release & License (“Consent”) is made by the parent or legal guardian (“Parent”) submitting artwork on behalf of the minor child (“Child”) in connection with participation in The Life is Good Kids Design-a-Thon (the “Promotion”), sponsored by The Life is Good Playmaker Project, Inc. (“Sponsor”).

 

1. Parental Authority

I represent and warrant that:

  • I am the parent or legal guardian of the Child whose artwork I am uploading, or otherwise have full legal authority to act on the Child’s behalf; and
  • I have the right to grant the permissions and licenses set forth in this Consent.

2. Artwork Ownership & Originality

I represent and warrant that:

  • The artwork submitted in connection with the Promotion (the “Artwork”) is original and created by the Child;
  • The Artwork does not infringe any copyright, trademark, right of publicity, or other intellectual property or proprietary right of any third party; and
  • The Artwork does not contain offensive, obscene, or inappropriate content.

 

3. License Grant

I hereby grant to Sponsor and its affiliates, partners, licensees, and service providers a perpetual, worldwide, royalty-free, irrevocable, and sublicensable license to reproduce, modify, adapt, display, publish, distribute, and otherwise use the Artwork and related submission materials, in whole or in part, in any media now known or later developed, including for:

  • Product creation and fulfillment
  • Marketing, advertising, and promotional activities
  • Fundraising and charitable initiatives
  • Storytelling, social media, websites, email, retail, and print materials

 

Sponsor may resize, crop, color-correct, or otherwise adapt the Artwork as reasonably necessary, while preserving the spirit of the original submission.

 

4. Use of Child Information

I consent to Sponsor using limited identifying information voluntarily provided by me (such as the Child’s first name and, at my discretion, age, hometown, and artwork title or description) in connection with Promotion-related marketing, storytelling, and communications. I will not provide, and Sponsor will not knowingly accept, any additional identifying information. Sponsor will not publish the Child’s contact information, or other sensitive personal data.

 

5. Publicity Release

I grant Sponsor the right to use the Artwork and permitted Child information for publicity, promotional, and fundraising purposes without additional approval or compensation.Participation does not guarantee that the Artwork will be selected, produced, or featured.

 

6. Release

I release and hold harmless Sponsor from any claims arising out of the use of the Artwork or submitted materials as permitted by this Consent, except to the extent caused by Sponsor’s gross negligence or willful misconduct.

 

7. Communications

I understand that Sponsor may contact me regarding the Promotion, including transactional and program-related communications. I certify that I have not provided any information that could result in communications being sent to or received by the Child.

 

8. Acknowledgment

By submitting the Artwork and checking the acknowledgment box, I confirm that I have read, understand, and agree to this Consent, and that writing my name in the web form serves as my signature.
